What to Expect on Your First Visit
Once the decision is made by you and your doctor to get home health services, Country Home Healthcare, Inc. will have staff come to your home for an assessment of your needs and medical condition. We will work with you and your doctor to set up a plan of care, including all of the services, treatments and equipment that you will need.
Our staff will continue to make regular visits to your home to carry out the plan of care, which is reviewed on a regular basis by the home health agency staff and your doctor. If your health problems or condition change, we will coordinate with you and your doctor to make any needed adjustments to your plan of care. Throughout their visits, we will teach you and your caregivers to continue any care you may need, for example wound care, therapy, and disease management and we will teach you how to recognize problems and potential side effects, and know what to do or whom to contact if they happen.
When you have met established goals, your doctor feels it is appropriate, and/or you are no longer eligible, your home health services will be discharged. Your doctor may want you to continue with treatment at an outpatient facility or with regular follow up appointments with them, other doctors, or we can even offer Private Duty Services!
